Title: No Quest Rewards

Hs anyne had a problem not receving the quest item reward when they complete the quest.  I get the experience reward but not the item reward.  This includes the Stronghold rewards such as the sword of backstabbing from Renal and the Mace for the Priest, along with the others, plus not getting the gems from Minsc's quest in unfinished business.  I thought someone might know if there is a specific module tha could cause a problem.  Thank-you for any help.
