DELETE_, CLONE_EFFECT can misindex files

Logic fail on my part. If you invoke either function with the check_headers = 0 argument, they completely ignore abilities to the point that they also don't update effect indices to account for new/deleted global effects. Easy workaround for now is to not use this argument, and fixed macros follow.
