You should try the Prophet module series for NWN. Or the Maimed God's Saga and Ravenloft modules for NWN2. I think those are the only remotely "Gothic" RPGs available (and they are great!). Of course, there are always The Witcher and Gothic series, but I would presume you know those already.

I was wondering... Is there other rpg with a gothic horror ambiance like icewind dale? With real npc in the group (like npc mod) but with this lonely feeling?
I really liked in iwd that feeling. We are alone, our only contact with civilisation is a peaceful (most of the time) little village in lost in the middle of nowhere (kuldahar).
Little npc alive to meet outside of Kuldahar (most of our interraction, slave a part, are ghost, zombis, squeletton... shadows...).
I loved baldur's gate, planescape, pillar of eternity, tyranny and other dragon age, but icewind dale was really the only game with that ambiance (I'm hoping for a rpg in the shadow of esteren univers, it has the same potential).

But otherwise, this mod was really (and is still) good. A real group was really what was missing in this game. And your npc are just great.
